After its formation in 1995, the OCCSW founded the Okaloosa County Women’s Hall of Fame to honor the women of Okaloosa County - past and present - whose contributions to the arts, athletics, business, education, government, the humanities, philanthropy and science, have been of great value to our community’s development. Each year's inductees are honored on a wall in the Okaloosa County Administrative Annex in Shalimar, FL.
The Hall is a monument to exceptional women in our county’s history.
APPLICANTS MAY CHOOSE UP TO TWO OF THESE CATEGORIES TO BE CONSIDERED FOR :
CAROLYN SPENCER HUMANITARIAN INDUCTEE - A woman who has made significant contributions to the welfare of the human and/or animal populations of Okaloosa County.
CONTEMPORARY INDUCTEE - A woman who has made significant contributions to Okaloosa County within the past 10 years
PIONEER INDUCTEE - A woman who has been a trailblazer for women in Okaloosa County or has made significant contributions to the founding and/or growth of Okaloosa County.
EDUCATION INDUCTEE - A woman who has made great contributions to the education field or received high recognition in Okaloosa County. This is a woman who has gone over and above in her education career.
MILITARY INDUCTEE - A woman who as a civilian has contributed greatly to our military community here or a woman who has had an exemplary career in the military showing great leadership during her service & is now a permanent resident of Okaloosa County.
BUSINESS LEADER INDUCTEE - This woman has grown a significant business in Okaloosa County, has contributed to the growth of women as entrepreneurs, climbed to the top of her career field or held a high leadership position in her company.
MATTIE KELLY CULTURAL ARTS INDUCTEE - This woman will have contributed significantly to the arts in Okaloosa County.
COMMUNITY SERVICE INDUCTEE - This woman has given extraordinary commitment and service to our Okaloosa County community
POSTHUMOUS INDUCTEE - this woman can be from any of these categories who contributed greatly to women of Okaloosa County and is now deceased.

1. Nominee Biography: Provide biographical information on the nominee, not to exceed two pages / 600 words per page max.
2. Summary of Qualifications: Provide information on the nominee’s specific qualifications for the Hall of Fame, not to exceed two pages. Please address how your qualifications in the any of the areas below (AS THEY RELATE TO OKALOOSA COUNTY), relate to one or both of the categories you have selected from the list above (in purple) that you would like to be considered for:
a. Professional/Business Qualifications and Contributions (e.g., starting a business/success in your career/any certifications received, awards received, etc)
b. Volunteer/Community Service (e.g., work with local charities or churches)
c. Woman-Specific Contributions (e.g., volunteer time/professional services provided to organizations that serve women and/or address women’s issues IN OKALOOSA COUNTY)
d. Involvement with Professional/Civic Organizations (e.g., Chamber of Commerce, Florida Bar Association, Women’s Clubs, Rotary, Kiwanis - business organizations, etc.)
e. Leadership Roles in the Community (e.g., spearheading a local project, serving as an officer in a professional/civic organization, or serving in an organization that relates to your career field, etc.)
f. Other (e.g., unspecified outstanding contributions to the wellbeing of Okaloosa County women, awards, notable “firsts” for women, etc.)

4. Letters of Recommendation: If desired, you may submit up to three ONE PAGE EACH MAX letters supporting your nominee.
